From the moment the Prime Air 767 cargo jet went off the end of the runway in Miami, killing five, it didn't take NTSB investigators long to piece together the timeline that led up to the fatal accident.

Even before they successfully recovered the cockpit voice recorder (CVR) and flight data recorder (FDR), the evidence was mounting, and would later be confirmed: this was a pure and simple (and avoidable) case of pilot-error.
